Inclusion Officer – Sheffield

Salary: £12ph+

It is essential that the candidates have experience of writing high quality Educational, Health and Care plans.

Job Purpose

To undertake statutory requirements relating to Children and Young People with Special Educational Needs and Disabilities (SEND) through appropriate assessment and review processes for work area
To gather information and advice from agencies involved with identified children and enable the council to carry out its statutory duties relating to SEND. 
To work closely with parents, education providers and other services across Education, Health and Care to ensure effective assessment and reviews are in place to meet an invidivual child’s needs
To be part of an integrated 0-25 SEND service meeting the needs of children and young people within the caseload
To advise education providers within the statutory framework of the Children and Families Act 2014, and with regard to the SEN Code of Practice 2015.
Duties and Responsibilities

To case manage the assessment and review process for an area in line with relevant legislation for a designated caseload in line with local policies and procedures to ensure that the Local Authority is fully compliant.
To ensure that Assessments for individual children and young people are carried out within statutory timeframes.
To ensure that high quality Plans are written and issued which are effective in improving outcomes for those with SEND who require provision to be made via a statutory plan. To make judgements about needs and provision to be included in the plan in light of advice from the young person, their family and from education, health and care professionals.
To conduct the timely review of Plans annually for the designated caseload and represent the Local Authority at Review meetings, as necessary. To ensure that where a Plan is no longer required this is identified and actioned through Review.
To ensure that Review documentation is received for all children and to follow up when records indicate these have not been received or progressed. To progress any request for changes in provision as part of the review.
To consider the provisions in the Plan and make decisions re whether the child is making progress in line with expectations and to then advise others accordingly. 
To ensure that reviews address all areas of work across 0-25 SEND so that planning is co-ordinated and aligned. To represent other 0-25 SEND services as required within those reviews and provide appropriate advice and guidance. 
To present cases for consideration of specialist provision and decision making to the appropriate decision making panel/body.
Ensure that provision is arranged for children following initial assessment and review, including liaison with other services across Education, Health and Care and with the family.
To work with children, young people and parents to ensure that casework is completed in a timely manner that addresses concerns in a way that ensures that there is a strong customer ethos.
Respond to issues raised by parents through casework and escalate to managers where there is a requirement for dispute resolution and mediation services. To work with Managers in response to complaints and legal action.
Ensure that transition plans to prepare children for adult life are devised and reviewed annually for young people, liaising as necessary with appropriate services.
To work with business support staff to ensure that appropriate administration of plans is carried out in line with statutory duties.
To liaise with parents, head teachers and other agencies on matters relating to the assessment and review process, including communicating decisions made and proposed amendments following the issue of the plan. 
To apply a solution focussed approach to facilitate negotiation where there are discrepancies between the needs and provision.
To participate in regular and minuted supervision, appraisal and access appropriate training.
To provide excellent customer service, responding to enquiries from internal and external service users, including parents / carers, other council departments, and external organisations within corporate timescales.
To use the Council’s record systems on a daily basis to update records and to progress and oversee casework.
To respond to telephone and written enquiries in a timely manner using discretion in replying so that consistent advice is given within agreed policy. 
To respond to general queries about the Assessment and Review process, including issues related to funding, timescales and available provision.
To provide general advice and guidance about SEND related matters to parents, education providers other professionals and agencies.
Knowledge and Skills

Continually update own knowledge of relevant developments in SEND nationally, regionally and locally and cascade relevant information to others. 
To work at all times within the Sheffield Safeguarding Children’s and Adult’s Boards procedures and to promote the welfare and safety of children and young people at all times.

To undertake other duties & responsibilities commensurate with the grade and level of the post as required.
